To work as a registered nurse in USA, the applicant must be licensed in the state where he/she is going to practice. Initial licensure in a state is awarded by receiving a passing grade in the NCLEX exam. NCLEX (National Council Licensure Examination) is designed to make sure that each nurse has the knowledge to practice safely at a minimum of an entry level registered nurse. This examination is application-based.

The NCLEX® examination is designed to test knowledge, skills and abilities essential to the safe and effective practice of nursing at the entry level. NCLEX examination results are an important component used by boards of nursing to make decisions about licensure. Only boards of nursing can release NCLEX examination results to candidates.

The exam contains multiple choice questions depicting clinical situations that an entry level nurse might face. Other requirements for licensure may differ from state to state.
